Announcement for our patients: Urban Health Initiatives

We are very sorry to announce that after struggling financially for the past year, the UHI board of directors has determined that we cannot hope to regain our financial footing. They have filed for Chapter 7 Bankruptcy effective 3/24/15.

Eileen K. Carpenter, MD is now with Spruce Internal Medicine Associates, part of the Clinical Care Associates of the University of Pennsylvania. Their office is at 800 Walnut Street, 16th Floor, Philadelphia, PA 19107, two blocks from Pennsylvania Hospital. The phone number is 215-829-0101.

Myron L. Sewell, MD is now seeing patients at Reed Street Health Center, 1325 S. 33rd St., 3rd Floor, Philadelphia, PA 19146,  the new South Philadelphia location of Spectrum Healthcare. The phone number is (215) 471-2761. He has always had another office in Burlington, NJ as well.

Maurice N. Daniels, MD has just joined the office of Dr. Patricia Purnell, 1508 Pennsylvania Avenue, Wilmington, De., 19806, Phone (302) 428-1142. He continues to work at the West Philadelphia office of The Pediatric and Adolescent Medicine Center of Philadelphia,  5249 Cedar Ave Ste C, Philadelphia, PA 19143-1524, 215-472-5437.

Linda Martin, MD is working with Greater Philadelphia Health Action, South Philadelphia Health Center on the second floor of 1408 S. Broad St., our previous location. The phone number is still 215-755-0700.

Noble S. Jones, MD is working for Urban Solutions on the first floor of the previous location. You may contact Urban Solutions at 215-755-6462.

The patient records have been purchased by Spectrum Health Systems. If you are seeing Dr. Sewell or one of the other providers at Spectrum's locations, you don't need to do anything. If you are seeing one of the other doctors, you will have to request your medical records be transferred.   Every effort will be made to make sure patients and their providers continue to have access to their health records. Please be assured that we want to continue to care for all our patients. You are wanted, very much, perhaps too much.  Patients will have more choices of providers, not fewer, once the dust has settled. We apologize for the inconvenience during our troubles.

Health Associates of South Philadelphia is now Urban Health Initiatives

As Health Associates of South Philadelphia, we've been providing high-quality medical care to a diverse population at 1408 S. Broad Street, Philadelphia, since 1992. Prior to that, most of our providers were part of Lomax Health Systems at the Point Breeze and Grays Ferry locations.

However, in 2011 we took on a new name and a new mission. Urban Health Initiatives is a Federally Qualified Health Center "look-alike" office, offering services to uninsured patients for a sliding-fee scale.

We still offer the same appointment-based care, and we are expanding our information technology capabilities to improve quality of care and patient access.

We look forward to announcing new services and locations in the future!
